How to annex a city from someone else.

Please explain steps on how to annex a city under present conditions. It seems to be different each time I try to occupy a city to be conquered.

You can occupy a city with a Diplomat/Spy unit only.

It is not that simple. Many times there is a response, such as "The workers rebelled. They say "aauuuuuu".

What is the actual process?
